Student-Athlete Tryout/Physical Information

In order to participate in an open try-out for a varsity athletic team sponsored by Western Michigan University Division of Intercollegiate Athletics, you will be required to complete the following forms to be cleared by the medical staff.

1.       Physical Exam Form

2.       Insurance Information Form

3.       Sindecuse Information Sheet (procedures for obtaining services at health center)

Please download and print the forms. Complete the medical history section and have the attending physician complete the rest of the form. Once you have completed the required forms, you must submit them to the team athletic trainer prior to the tryout date. Without completed forms, participation will not be granted.
